Our sign-up process is simple.
1. Call or email us to receive a copy of the Authorization Agreement and Industry Standard Questionnaire. (see 'Contact Us' for phone number and email address)
2. Print and fill out the Authorization Agreement and Industry Standard Questionnaire. Fax or mail to Mandlik & Rhodes. (see 'Contact Us' for address and fax number)
3. MRIS Customer Service will contact you within 1 business day of receiving completed forms and provide you with a manifest form.
4. After you have been accepted fill out the MRIS Coupon Shipping Manifest and send to us with your coupons (use the mailing address on the manifest).
